The NHS in South West London have developed a new model of care that provides Croydon care homes with the intelligent Camascope (formerly named VCare Systems) Remote Patient Monitoring system and access to a 24/7 virtualised NHS clinical support service delivered by Immedicare.
The Camascope Remote Patient Monitoring system can automatically monitor a resident’s health and calculate their national early warning NEWS2 score. If the resident’s NEWS2 score increases, the Camascope system will trigger alerts and provide carers with messages guiding them on the next steps that need to be taken. Messages include a prompt to make a video call to the Immedicare Digital Care Hub where NHS clinicians can assess a resident’s condition and provide ongoing clinical support.
The new service model will help care homes to recognise when a resident may be deteriorating or at risk of physical deterioration and provides carers with immediate video access to NHS clinical support 24 hours a day, 7 days a week.
A great example of a combined technology and virtual clinical service model helping to support the principles of right care, right place, right time.
